Altar of Victory: Salvation/Rededication Form

So, you are reading this form because you felt compelled to either accept Jesus as your Lord and Savior OR you are re-committing your life to Him! That's WHAT'S UP & welcome to the family!

But don't just stop there. Take a moment to fill out this form. It is important that you are equipped with resources and relationships as you embark on this new journey with Christ. We want to help provide that for you. But we cannot do that if you don't fill out the form. We promise you that we will not harass you! We also promise to be there and walk with you and encourage you in this new journey, if you let us.

If you have never accepted Jesus Christ as your Savior, or do not know what it means to “accept Him into your heart”, then please read the following simple steps to salvation.

> First: Admit your need (“I am a sinner. I have done wrong things in my life. I haven’t been perfect Lord”).
> Second: Be willing to turn away from your sins / wrong-doing (Repent).
> Third: Believe that Jesus died for you on the cross & has risen from the grave.
> Fourth: Through prayer, ask Jesus Christ to come in and control your life (invite Him into your heart).
> Finally: Receive Him as Savior and Lord. It’s that simple. God loves you!

If you followed these steps just now, then God’s Word (the Bible) says that you are saved!
Prayer of Salvation

God, I repent for my sins. I thank you for the gift of your son Jesus Christ. I acknowledge that he died on a cross, so that I might have eternal life with you, and I choose to accept that gift today. I ask you to come into my heart, forgive me of my sins, and accept me as one of your own. I make a decision today to turn away from the world, and anything that does not please you. Change me, shape me, and mold me, I place myself completely in your hands. In Jesus name. AMEN

1 Corinthians 15:57
"But thanks be to God, who gives us the victory through our Lord Jesus Christ..."
